Shared follow-up and survivorship care for low-risk endometrial cancer: Shared Care Plan This is an individualised care plan that contains the key follow-up elements and schedule required to provide ongoing comprehensive care to a woman who has received treatment for low-risk endometrial cancer. The Shared Care Plan is to be completed in collaboration with the woman, her specialist team and her General Practitioner (GP). The plan will become active after review and agreement by the woman, GP and specialist.
